Infection is a gameplay mechanic used to limit the player's health and restrict decision-making. Infection is accumulated through infectious fog, Spitters, and syringes, and is represented by a green bar above the health bar on the player's HUD. Infection resistance can be improved with boosters.
Mechanics[]
Players start with no infection, and can have a maximum of 99% infection.
The player's maximum health is the inverse of the level of infection. For example, a player with 70% infection will have a maximum health of 30%. The player's health will slowly decline until it reaches the new maximum.
Infection is increased from infectious fog, Spitters, and syringes up to 99%. Whilst infection cannot directly kill the player, it can leave the player with limited health. When attacked by spitters, infection will raise up to 13%, depending on the distance.
As long as the infection level is not rising (i.e. from infectious fog), it will slowly decline to a maximum of 85%.
Infection causes the player to see visual distortion and green drops on their mask and to cough and breathe heavily.
To remove infection, players can either use the disinfection packs found within lockers and boxes, or use the stationary Disinfection Station found within some expeditions.
Infectious fog can be avoided with Fog Repellers and Fog Turbines. Spitters can be avoided with C-Foam or Flashlights. Medical Packs are an alternative to I2-LP Syringes, and although weaker, boosters can substitute for IIx Syringes.

Whilst it is unclear, "Infection" likely refers to the virus and parasite outbreak within The Complex.
